Cancel / Reschedule Appointment Notification

It is always recommended that any Appointment Owner, or where relevant their designated Attendee, uses the Cancel / Reschedule Appointment Notification form below to contact Focused Therapy London if they need to Cancel or request to Reschedule any Confirmed Appointment or Requested Appointment, as the below form requests all of the required information necessary to do so, as summarised within the Cancellation / Rescheduling Policy Summary.

Please always ensure to list under the Any Additional Information section of the form below, which will only appear as you are completing this form, any circumstances under which the relevant Appointment should not be Cancelled or Rescheduled as requested, should any such circumstances apply, as otherwise the relevant Appointment will automatically be removed from Focused Therapy London's Appointment schedule and will not be able to be attended where it has already been scheduled as a Confirmed Appointment; or, will stop being processed any further where it relates to a Requested Appointment.

Standard Contact Times

Please be aware any Cancel / Reschedule Appointment Notification will be processed from the time it is received by Focused Therapy London, but will usually only be responded to during the following Standard Contact Times.

Monday to Friday: 11am - 7pm
Saturday: 12pm - 6pm

The only circumstances where Focused Therapy London will usually provide a personalised reply outside of the above Standard Contact Times, later on the same evening or over the weekend after 6pm on a Saturday, will be where the Confirmed Appointment or Requested Appointment's relevant Cancellation / Rescheduling Notification Period deadline is shortly due to or has already passed; or, under any circumstances where Focused Therapy London did not have sufficient time and/or capacity to reply any earlier on the day that the relevant Cancel / Reschedule Appointment Notification was received within the above Standard Contact Times for any reason.

SHOULD YOU NEED TO CANCEL OR RESCHEDULE EITHER A REQUEST APPOINTMENT OR A CONFIRMED APPOINTMENT WHILST THIS FORM IS OFFLINE, YOU ARE STILL ABLE TO DO SO IN WRITING VIA ANY OF THE METHODS DEAILED BELOW:

Alternate methods to Cancel or Reschedule an Appointment other than by the use of this form:

  • send an email to Focused Therapy London's general use info@focusedtherapylondon.com email address, with the subject of the email typed in capitals as follows CANCEL / RESCHEDULE APPOINTMENT NOTIFICATION; or
  • reply to the relevant Appointment Confirmation Email or Confirmed Appointment Notification received directly from Focused Therapy London where a Confirmed Appointment has already been scheduled; or
  • reply to any relevant personalised communication received directly from Focused Therapy London in relation to the scheduling of the Requested Appointment (not including any 'noreply' email address which is used to provide automated responses only and is not monitored); or
  • send a text message to Focused Therapy London's mobile phone number where this has already been provided (usually listed within any email footer provided within a personalised, non-automated, email response sent directly by Focused Therapy London in relation to a Requested Appointment; and, which can always be found in the email footer of any Appointment Confirmation Email received directly from Focused Therapy London)



Your Cancel / Reschedule Appointment Notification must clearly state the following information:

  • the full name of the Attending Client (as requested or already scheduled)
  • the Appointment Date (as requested or already scheduled)
  • the Appointment Start Time (earliest requested (where more than one was provided) or already scheduled)
  • the Appointment Length (as requested or already scheduled)
  • whether this Appointment needs to be Cancelled or Rescheduled (either now or at a later time or date)
  • whether there are any circumstances under which this Appointment should be kept as previously requested or already scheduled (optional)

APOLOGIES FOR THIS TEMPORARY INCONVENIENCE

What happens next

Focused Therapy London will reply to the submitter to provide a personalised response in relation to any Cancel / Reschedule Appointment Notification received from them; and, where relevant additionally provide any scheduling feedback where the Appointment has been requested to be Rescheduled now rather than at a later time or date.

Where any Confirmed Appointment or Requested Appointment has been requested to be Cancelled or Rescheduled by the Appointment Owner's designated Attendee (where relevant), and in the process the Appointment Owner needs to be made aware of any relevant Refund due to them or payment due from them where any Late Notification Cost applies, Focused Therapy London will additionally contact the relevant Appointment Owner to notify them accordingly.
